Our benefits objective is to provide a competitive and cost-effective program offering choice, quality, and value to our employees and their families. Because care is important, coverage begins on the first day of the month following date of hire.
The medical plan is a consumer-directed healthcare plan with a local and national network. It offers an affordable alternative to traditional healthcare plans. The plan engages employees in their healthcare decisions and spending. Upsher-Smith subsidizes a significant portion of the premium. Employees may elect Employee, Employee +1, or Family Coverage.
Upsher-Smith offers two dental plans which allows for a plan that best fits the employee’s needs. Both plans cover preventive care, basic restorative and major restorative services. Orthodontia coverage is available for both children and adults.
The Vision Plan covers eye exams, frames, lenses, and contact lenses. Employees pay the full cost of this benefit and may elect Employee, Employee +1, or Family Coverage.
Group Life Insurance
Upsher-Smith provides a basic life insurance benefit of 2 X annual salary (minimum/maximum coverage levels apply). The plan also provides Accidental Death and Dismemberment Coverage in the amount of 2 X annual salary.
Elective life insurance may also be purchased. The cost of coverage is based on age.
Dependent Life Insurance
Life Insurance coverage may be purchased for spouse and dependent children. Employees pay the full cost of this coverage.
Disability coverage provides a specific amount of replacement income should an employee become disabled and unable to work. Short-term and long-term disability coverage is available.
Maximum benefits coverage limits apply. The employee is responsible for the full cost of this benefit, but Upsher-Smith will increase the employee’s income by the amount of his/her long-term disability premium to help offset the cost of this benefit.
Flexible Spending Accounts
The Healthcare Account allows payment on a pre-tax basis for certain medical/dental expenses not covered by the Benefit Plan. The Dependent Care Account sets aside money on a pre-tax basis for certain dependent care expenses. Maximum contribution limits will apply.
Immediately upon hire, participation in our 401(k) Retirement Plan is encouraged. This plan is designed to help employees save money for retirement. It allows pre-tax employee contributions of up to 100% of annual salary. Upsher-Smith provides a generous matching contribution.
Upsher-Smith also offers a Profit Sharing Plan. Contributions to this plan are based on the profits of Upsher-Smith. The Profit Sharing Plan is 100% company funded.
Profit Sharing contributions begin the first quarter after 1,000 hours of service.
Paid Time Off and Holidays
Three weeks of Paid Time Off (PTO) is available during your first year of service. Forty hours are granted immediately and upon the 1 year anniversary. Remaining hours are accrued throughout the year. Additional PTO will be earned at the beginning of 3, 5, 10, and 15 years of service. Upsher-Smith provides 12 paid holidays, which are designated prior to the start of each calendar year.
