Coverage begins on the first day of the month following your date of hire
Medical
Upsher-Smith subsidizes a significant portion
of the premium. You may elect Employee,
Employee +1, or Family Coverage.
The medical plan is a consumer-directed
healthcare plan with a local and national network.
(A pre-existing condition provision may apply.)
Dental
The Standard Dental Plan provides 100%
coverage for preventive services without a
deductible, 80% for basic restorative, and 50%
for major restorative services (after a deductible),
and dependent orthodontia coverage.
The Comprehensive Dental Plan provides the
same level of coverage for preventive services
and basic restorative, 60% for major restorative
services, and orthodontia coverage for all
covered persons.
Vision
The Vision Plan covers eye exams, frames,
lenses, and contact lenses. You pay the full
cost of this benefit and may elect Employee,
Employee +1, or Family Coverage.
Group Life Insurance
Upsher-Smith provides a basic life insurance
benefit of 2 times your annual salary (minimum/
maximum coverage levels apply). The plan also
provides Accidental Death and Dismemberment
Coverage in the amount of 2 times your annual
salary.
You may also purchase elective life insurance for
yourself. The cost of coverage is based on your age.

Dependent Life Insurance
You may purchase life insurance coverage for
your spouse and dependent children. You pay
the full cost of this coverage.
Disability Insurance
Disability coverage provides you with a specific amount of replacement income if you become disabled and are unable to work. You may purchase short-term and long-term disability coverage.
- A waiting period of 7 calendar days will apply before short-term disability payments begin, with a maximum of 165 days of coverage
- Long-term disability benefits are payable on the 181st day of disability and may continue to age 65
Maximum benefits coverage limits apply. You are responsible for the full cost of this benefit but Upsher-Smith will increase your income by the amount of your long-term disability premium to help offset the cost of this benefit.
Flexible Spending Accounts
The Health Care Account allows you to pay for certain medical/dental expenses not covered by your Benefit Plan on a pre-tax basis.
The Dependent Care Account lets you set aside money on a pre-tax basis for certain dependent care expenses.
Maximum contribution limits will apply.
Retirement Plan
You may participate immediately in the 401(k) Savings Plan. This plan is designed to help employees save money for retirement. It allows pre-tax employee contributions of up to 40% of your salary. Upsher-Smith will match 50% of your contributions for the first 6% of your salary.
Upsher-Smith also offers a Profit Sharing Plan. Contributions to this plan are based on the profits of Upsher-Smith. The Profit Sharing Plan is 100% company funded.
Employer contributions, including employer matching contributions and Profit Sharing contributions, begin the first quarter after 1,000 hours of service.
Paid Time Off and Holidays
You will receive 3 weeks of Paid Time Off (PTO) during your first year of service. Forty hours are granted immediately and you accrue the remaining hours throughout the year. Additional PTO will be earned at the beginning of 3, 5, 10, and 15 years of service. Upsher-Smith provides 11 paid holidays, which are designated prior to the start of each calendar year.
